Mind My Health is a free online platform tool that allows users to engage with their loved ones and chosen decision makers, discover organ donation options, and securely upload and store advance care planning documents. Work on this innovative project began in 2015 as an idea from leaders at The Carolinas Center for Hospice and End of Life Care through funding support from both The Duke Endowment and The North Carolina License to Give Trust Fund Commission. It is now a cornerstone project of Health Sciences Health Innovations Group.

The North Carolina Serious Illness Coalition is the evolution of the work done in 2019-2020 by the NCIOM Task Force on Serious Illness Care. The NCSI Coalition is a continuation of statewide partnerships to advance the 13 recommendations made in the Task Force’s final report. The mission of the NCSI Coalition is to engage a broad range of stakeholders and their perspectives, including providers, consumers, policy advocates, industries, and others, to accelerate the solutions needed to achieve the desired vision for serious illness care.
Financial Support Provided By
HSHIG has received a significant technology grant in 2020 from the State of NC’s License to Give Trust Fund Commission. This grant is focused on developing new capabilities for communicating advance health care documents “wherever and whenever they may be needed” for individuals who cannot speak for themselves.
